In filling out this form you acknowledge:

  1. To the best of my knowledge, entrants are not related to FOLMC Staff or Board Members; contest Judges; or contest Administrators.
  2. Students have agreed to participate in the Friends of the Library, Montgomery County (FOLMC) Mosaic Contest. Student certifies that they are the original author of this work and agree to permit the publishing of this work in a student anthology if selected.
  3. I further verify that I have permission from my students’ parent/guardian to submit this work for the contest.
